I understand that forfeits are a very undesirable part of the Intramural Sports Program. Participants lose the opportunity to play, valuable facility space is wasted, and money is spent to pay employees. Therefore, as the representative for the team listed above, I understand that it is my responsibility to read and know all Intramural Sports Policies and Procedures as outlined in the Rhodes Intramural Handbook. I will also ensure that my team will have the minimum number of players available prior to the contest's scheduled starting time. Furthermore, I understand that the forfeit policy will be in effect for the following situations:
1. Failure to have the minimum number of players necessary for competition in that sport of event (as designated in the rules specific to that sport.) 2. Use of an ineligible player by the team in question.
In addition, I understand that, should my team forfeit twice in a season, we may be eliminated from further competition in that sport. After each forfeit, a $20.00 forfeit fee is due to Recreational Services to cover the cost of the contest. I am also aware that I may avoid paying a forfeit fee by properly filing an advance default. Procedures for this are in the Intramural Sports Handbook.
I understand that it is my responsibility to secure all scheduling information regarding the games of my team and that the Recreational Services office is not responsible for notifying you of scheduled contests.
I understand that all basic equipment, aside from personal equipment, will be provided to my team for each contest. I further understand that as captain of the aforementioned team, I assume responsibility for the proper care and return of said equipment. If equipment is damaged due to negligence on the part of my teammates, or if it is not returned following the completion of the contest, I assume financial responsibility for its replacement.
Checking the box below indicates that I assume financial responsibility for the forfeits of my team in this Intramural Sports activity. I agree that my team will participate in all regular season and playoff games, or I will properly notify the Recreational Services Office of my team's intent to default. I agree to assume full responsibility for the $20.00 forfeit fee should my team be assessed a forfeit for any of the reasons listed above. My check below further indicates that I assume financial responsibility for the replacement of equipment damaged through the misuse of my teammates and of lost equipment.
